Recent understanding of multiple sclerosis (MS) has increasingly emphasized the impact of silent progression on disability accumulation and long-term outcomes. Detecting and monitoring patients for subtle changes, particularly those related to progression independent of relapse activity (PIRA), is clinically challenging. However, recent advancements in evidence-based tools and assessments offer frameworks for clinicians to identify and manage patients at risk for silent disease progression. Additionally, growing evidence on disease-modifying therapies (DMTs) within progressive disease is positioned to support tailored treatment strategies, and incorporating shared decision-making (SDM) into treatment discussions ensures comprehensive care aimed at improving patients' quality of life.
